I hereby verify the following: 1) I am hereby authorized to submit this tryout form on behalf of my team, program, and/or organization; 2) any, and all, of the information submitted in the form above and fields contained therein, is true and accurate to the best of my knowledge under the penalty of perjury; 3) you acknowledge that your tryout may or may not be submitted and/or published; 4) The Baseball Network, Zachary J. Cook, and/or their affiliates, including, but not limited to their parent companies, subsidiaries, affiliates, successors, assigns, officers, directors, employees, agents, owners, and representatives (collectively, The “Company”), shall not be liable to any party, including any third party, for any indirect, incidental, consequential, special, punitive, or exemplary damages, including but not limited to lost profits, loss of business, or loss of data, arising out of, or in connection with, this Verification and the subject matter related thereto; 5) The Company reserves the right to refuse to accept and/or publish any tryout or other information submitted by any user at any time and at its sole discretion without a reason, justification, or explanation; and 6) by checking the box, you are confirming, acknowledging, and agreeing to the Verification and terms herein.
